The 2020 Middle School Soccer season will begin October 5th.  Forms will be distributed to those students interested in trying out at the beginning of September.  

Forms need to be turned into one of the coaches (Coach Seiler/Coach Underwood) before a student will be allowed to try-out.

Forms needed:

• Soccer Information Sheets (Will be passed out in September)
• Physical Form (signed by a doctor)
• Concussion Form
• Cardiac Arrest Form
